Forms can also be found in the entry of the church or by contacting the parish office by phone at (301) 539-3903 or by email. Once filled out, they may be dropped in the Offertory Basket during Mass or mailed or hand-walked to the parish office.
Please notify the office if you change your address or move from the parish.
